Non-Student Volunteer Application

Dear Volunteer Applicant,

We are pleased you have chosen to apply to serve as volunteer with Ascension Seton. Please take a moment to review all of the information below to ensure that you submit a complete application.

Volunteer applicants must:

provide a minimum of 1 year of service
serve for 3 - 4 hours each week (depending on service area and site)
be 18+
have a social security number

This application is a two-step process.

Step 1: Complete the below application and upload any available information.
Once submitted, you will be redirected to a link to Application Step 2.
Step 2: Click the link and complete the secondary form.
- You will be contacted by the Volunteer Services Office after the application window is closed.

Criminal Background Check

A clear criminal background check is required for all applicants. You will be receiving an email from our third party vendor. Please watch for an email and complete in full all requested information.

Please note that this application is created through RedCap, which is HIPPA compliant. Your personal information will be protected.

Once your background has cleared, Ascension Volunteer Services will be notified. All applicants must complete and clear the background check portion to be considered for service with any Ascension facility.

- Immunization Upload

Immunization records are required for all volunteer applicants to join the volunteer program within Ascension Seton.

Download the below document and take to your primary care provider (physician, nurse practitioner or physician assistant) to fill out and sign or stamp.

Should you need missing vaccines, your physician can perform blood draw to determine if any required vaccines are missing.

Proof of Measles, Mumps and Rubella vaccinations (2 doses, first dose must be on or after your 1st birthday)

Proof of Varicella vaccinations (2 doses) or documented history of chicken pox

Proof of 1 TB blood test (administered no earlier than 1 month from application submission)

Proof of a TDAP test in the last 10 years

Please note that if you have a prior false positive for a TB skin test, a chest X-ray can be accepted in combination with the false positive documentation.
