COVID-19 Vaccine:
Ascension Seton now requires ALL associates (including those employed by subsidiaries and partners; physicians and advanced practice providers, whether employed or independent; medical students and residents; and volunteers, observers, and vendors) entering any of our facilities to be fully vaccinated on or before November 12, 2021.
Please submit proof of your COVID-19 vaccination along with the other required vaccinations.
Face Masks:
Cloth or personal masks are no longer allowed on any associate, in the hospital, clinical or healthcare setting. Only masks supplied by the hospital can be worn. This includes students and contractors working on our behalf when they are within the hospital walls.
Thank you for your interest in an observation opportunity. There are many steps that must be accomplished to protect the observer, our patients, and the Ascension Seton/Providence Network.
Please complete all forms on the application and upload all necessary documents. There are several forms past the application cover sheet that need to be completed and can be viewed once you complete the application cover sheet and hit submit. Please thoroughly review the information as you will be held accountable if you violate HIPPA laws...your signature implies you have read and understood the material.
It is the responsibility of the Observer (you) to arrange a Sponsor who agrees to take responsibility for you during your Observation. You must be at least 18 years of age to be an Observer and fall under one of the categories below.
An observer must meet the following requirements:
- Not currently enrolled in High School
- A physician who is not credentialed through the Medical Staff Office
- A physician resident who is not employed by Ascension Seton
- An Associate of Ascension who is not employed by Seton/Providence
- Not a current volunteer (must reach out to volunteer services)
IMMUNIZATIONS:
We will also need proof of the following immunizations:
Ascension Seton/Providence Policy requires the following immunizations in order remain in compliance. Failure to submit proof of the required immunizations below will delay processing and your application will be considered incomplete.
A. Two Varicella vaccinations, a documented history of disease, or a positive titer proving immunity
B. Two Measles, Mumps, Rubella (MMR) vaccinations or a positive titer proving immunity for each disease
C. Three Hep B vaccinations or a signed declination
D. TB Test within the past 12 months to include date administered, date read, and results in mm...
E. Influenza vaccination (if Observation is during flu season: usually September through April)
F. Tdap (pertussis vaccine)
G. COVID-19 (fully vaccinated on or by November 12, 2021)
BACKGROUND CHECKS:
Ascension Seton/Providence require $40 to process your background check
If you are an international applicant, the cost of the background check will be more like $150-$200+ and take about 3-4 weeks to process. Notify the Observer Coordinator at Academicadmin@ascension.org of the country, and he or she will let you know how much the cost will be. Background checks for international requests take about 3-4 weeks to process.
Once your vaccine requirements are verified and your signed sponsor form is received, you will be contacted by our Academic Administration Team with instructions on how to pay for the background check fee.
RULES TO REMEMBER
- Observers must remain with their sponsors at all times. Your sponsor can NOT delegate their sponsorship to any other individual.
- Observers must have their signed sponsor form with them at all times.
- Observers in the operating room must present a signed sponsor form to the surgery director prior to observation. The OR staff must verify the observers clearance status prior to allowing observation.
