Your position in the program is contingent upon completion of an Occupational Health Clinic clearance and completion of a background check. As a reminder, the occupational health requirements are listed here:
MMR - Measles, Mups, Rubella
Varicella - Chicken Pox (either history of disease or proof of vaccination)
TB (tuberculosis) must be a TB blood draw - QFT Gold of Tspot within 90 days of start
Current Season Flu Shot
TDAP within last 10 years
Urine Drug Screen (to be completed at one of the Occupational Health Clinics)
To complete the occupational health clearance, please gather any immunization records you may have from your primary care physician for the above immunizations. Below are the instructions to set up a ReadySet account. Please attach your records to that account. If you have your COVID vaccination record, they will want to see that as well (this is no longer required, but is being tracked by our nurses). Our occupational health team will handle any vaccinations/ tests you do not have records for at no cost to you!
Once you have created your ReadySet account, please email me back with three weekday/business hour times that work to schedule your Ascension Occupational Health clinic appointment. I will email you back with the day and time of your appointment. The clinic is open Monday through Friday 8am to 3pm. Clinic appointments book up very quickly and are first come, first served.

**Please read this thoroughly before the date of your appointment
to ensure you are fully prepared for your visit. **
You will need to set up an account with Occupational Health prior to
your appointment:
1. Set up your ReadySet My Health account.
1. Go to ascension.readysetsecure.com
2. Click on "New User? Click Here to Begin"
3. Follow the instructions to register with Access Code: 3755
4. Your Program Type is Volunteer Onboarding.
You may need to use a laptop or PC; or rotate your
device sideways to view all buttons.
5. Once your account has been completed, you may refresh the
browser and log in with username and password.
2. On the left hand menu, navigate to "Health Surveys
• Complete all Health surveys in their entirety
This step may take 30-60 minutes.
• If you have questions related to your pre-
appointment health forms, the onboarding
requirements, or clinic location details, please
call the clinic directly. Contact information can
be found in the My Health portal.
3. On the left hand menu in ReadySet navigate to "Documents"
• Upload copies of any immunization record(s) and/or lab titers you
may have showing immunity for the following.
• Note: These can usually be found with your Primary Care Provider
or Student Health at school. In the event you are unable to produce
one or more of these records, titers will be drawn and/or
immunizations will be provided:
• Influenza, Seasonal COVID-19
• Measles, Mumps, and Rubella (MMR) Varicella
• Tdap (Tetanus, Diphtheria, and Acellular Pertussis)
Plan for the following on the day of your appointment:
• Urine drug screen
• Review of your immunization history; draw titers and/or immunize
when necessary
• Tuberculosis screening
On the day of your appointment, please:
• Plan to arrive 15 minutes prior to your appointment.
• If you are running late, please call the clinic directly. Contact
information can be found in the My Health portal.
• If you have arrived at the facility and are having difficulty locating
Associate Occupational Health, inform the front desk that you are
a new Ascension volunteer here for your drug screen.
• Bring a valid, government-issued photo ID.
• Bring copies of your immunization history and/or lab titers
showing immunity if you were unable to upload documents in the
ReadySet My Health portal in advance.
• Bring or wear your glasses or contacts.
• Drink plenty of fluids to ensure an adequate sample for your urine
drug screen.

• Fasting is NOT required. Please maintain your normal dietary and
medication regimen.
• Due to the nature of the visit, please do not bring children or other
dependents to your appointment. If you are a minor, you must
bring a parent or guardian to the appointment with you to provide
consent.

• In the event you need to reschedule, please contact Associate
Occupational Health at least one business day prior to your visit.
In addition to the Occupational Health clearance, all volunteers must pass a criminal background check. The email with information for the background check will not come until you have made your occupational health appointment.
