Student Volunteer Application (Step 1)

Dear Volunteer Applicant,

We are pleased you have chosen to apply to serve as volunteer with Ascension Seton. Please take a moment to review all of the information below to ensure that you submit a complete application.

Applications are open for student applicants on the following dates and will not be accepted outside of these dates.

Fall Semester- Application open July 1st- July 31th

Spring Semester- Application open December 1st- December 31th

Summer Semester- Application open April 1st-April 30th

Volunteer applicants must:

provide a minimum of two semester commitment
serve for 4 - 6 hours each week (depending on service area and site)
be 18+
have a social security number

This application is a two-step process.

Step 1: Complete the below application and upload all requested information.
- Once submitted, you will receive an email with a link for Step 2.
Step 2: Continue on the redirected link and complete the secondary form.
- You will be contacted by the Volunteer Services Office after the application window is closed.

Ascension Seton Volunteer Directors

Immunization Record Submission

Occupational Health Clearance is required for all applicants. This will require a doctor's visit. Please download and print the Immunization Document below to take to your physician visit.

You MUST have a medical provider or college health office complete the Immunization Document including their signature.

A complete immunization form includes:
- Legible contact information including your social security number lack of social security number will disqualify your application.

- Proof of Measles, Mumps and Rubella vaccinations (2 doses, first dose must be on or after your 1st birthday)

- Proof of Varicella vaccinations (2 doses) or documented history of chicken pox

- Proof of 1 TB blood test (administered no earlier than 30 days before application submission)

Note- if you have previously had a positive TB Skin test, you may submit a chest x-ray within the last year in combination with the positive documentation.

-Documented Flu vaccine for current flu season

-Proof of a TDAP test in the last 10 years

- A medical provider's name, signature and stamp at the bottom of the page must be provided

An incomplete form will automatically disqualify you from the application process. Records submitted on any document other than the provided form will also automatically disqualify you from this process.

Criminal Background Check

A clear criminal background check is required for all applicants. You will be receiving an email from our third party vendor. Please watch for an email and complete in full all requested information.

Please note that this application is created through RedCap, which is HIPPA compliant. Your personal information will be protected.

Once your background has cleared, Ascension Volunteer Services will be notified. All applicants must complete and clear the background check portion to be considered for service with any Ascension facility.

I understand that an incomplete background check form or incomplete/non-compliant immunization form will automatically disqualify me from the application process. I have read the instructions carefully and have taken care to follow them completely.
